Cynefin: why do things break and why is it hard to fix them?

I'll review things that broke for me in the last year: Python utilities I maintain, customer software, and infrastructure. I'll introduce and use the Cynefin framework to reflect upon those incidents and to select appropriate strategies to fix things.
